Akinetic crisis: causes and first aid.
With Parkinson's disease, a pathological condition can develop - an akinetic crisis. Symptoms are characteristic of an akinetic crisis:
- A sudden increase in hypokinesia - a sharp decrease in motor activity and a sharp development of slow motion;
- Increased tremor of the limbs;
- Strengthening muscle stiffness - increased muscle tone, characterized by stiffness of movement, muscle resistance to stress, up to immobilization;
- The development of dysarthria is a violation of speech function as a result of limiting the mobility of the lips, tongue, soft palate;
- The appearance of dysphagia is a violation of the act of swallowing.
The causes of an akinetic crisis may be related to:
- with a sharp discontinuation of the drug Levodopa, Nakom, Sinemet, Madopar;
- with a decrease in the dosage of Levodopa drugs and other antiparkinsonian drugs;
- with impaired absorption of drugs used in the treatment of parkinsonism;
- with cerebrovascular diseases;
- with an associated acute infection, complicating the course of the underlying disease.
With an akinetic crisis, first aid is provided in a hospital, after finding out the exact cause of its development.
If the cause of the akinetic crisis was the cancellation or reduction of the dose of Levodopa drugs and the like, then the treatment will be based on the resumption of taking medications or increasing their dosages.
In case of disorders of absorption of drugs through the gastrointestinal tract, parenteral nutrition is used. Additionally, intravenous Amantadine sulfate is prescribed (lengthens the action of Levodopa) 500 ml / 1-2 times a day for five to ten days and the subsequent inclusion of the drug in the course of treatment (100-200 mg three times a day).
If swallowing is difficult, drugs are administered in dissolved form through a probe.
An akinetic crisis is dangerous for the possible development of acute respiratory failure and / or heart failure, which will require the use of forced ventilation.
In the period after the relief of the akinetic crisis, massage and therapeutic exercises will be required to restore the performance of the muscular apparatus.
