Dermatitis Duhring.
Dermatitis Duhring (synonyms: dermatitis herpetiformis Duhring, Duhring disease) is a chronic recurrent skin disease of unknown etiology. It is accompanied by skin rashes similar to the skin manifestations of herpes simplex.
Signs of Duhring dermatitis: polymorphic skin rashes (in the form of small nodules, blisters, papules, erythema), accompanied by severe itching and burning. The rash is prone to an annular arrangement, the vesicles turn into pustules, self-resolve after three to four days with the formation of ulcerations and crusts, sometimes scars. After healing, areas of the skin are prone to pigmentation disorders (hyper- or hypopigmentation).
The rash can be located in various areas of the skin with the exception of the soles and palms, but the most characteristic places of the rash are the areas of skin where the limbs bend, the interscapular region, shoulders, lower back and buttocks.
The course of Duhring dermatitis herpetiformis has rather long periods of remission (up to a year). In the acute phase of the disease, in addition to uncomfortable skin irritation, there is a general deterioration in well-being, fever, insomnia, and nervousness.
Differential diagnosis of Duhring dermatitis is carried out with skin rashes of herpes simplex, toxicoderma, pemphigus, exudative erythema multiforme.
Diagnosis of the disease involves the Jadasson iodine test: when potassium iodide gets on the skin (in the form of a compress) or as a 2% solution into the digestive tract, new rashes appear after 24 hours.
Treatment of Duhring dermatitis is aimed at reducing the acute period of the disease, eliminating itching, and disappearing rashes.
An important role in treatment is assigned to a diet that excludes gluten-containing foods, cereals from wheat, barley, millet, oats, protein, iodine-containing foods. They carry out supportive therapy to normalize the digestive tract (in case of violations of enzymatic activity, dysbiosis), take vitamins of group B.
The main medicines are drugs of the sulfone group, antihistamines. For the early healing of rashes, antiseptics are used topically (furacilin, potassium permanganate solution, fucorcin, brilliant green). In severe cases of the disease, topical ointments and aerosols with corticosteroids are shown to relieve inflammation.
