Gynecology and Mammology >>>> Bartholinitis treatment
Bartholinitis treatment.
Treatment of bartholinitis is correlated with the complexity of the course of the disease. In the case of a spontaneous outcome of an abscess (cyst) or inflammation of the excretory duct of the gland, conservative drug therapy is chosen. In the case when the cyst or abscess continues to develop, increase in size, the inflammatory process affects the adjacent tissues, the patient's general condition worsens (fever, pain), priority is given to surgical treatment.
With inflammation of the excretory duct of the Bartholin gland, the following procedures are shown:
- Ointment applications on inflamed glandular foci (ichthyol ointment, Levomikol, Vishnevsky's ointment),
- Baths with the addition of antiseptics (chlorhexidine, furacilin, potassium permanganate, chlorophyllipt, miramistin) and herbal infusions,
- Lotions with 10% saline solution (done regularly, 6 times a day for 30 minutes),
- Cool compresses on the inflamed area,
- Anti-inflammatory drugs (ibuprofen),
- Painkillers (baralgin, analgin),
- Antimicrobial drugs are used as needed and in the presence of smear results for microflora: antibiotics (fluoroquinols, cephalosporins), antiprotozoal (trichopolum, tinidazole),
- Physiotherapy is indicated only after the extinction of the acute process and in the absence of temperature,
- Immunomodulators,
- Vitamin therapy.
The cyst of the Bartholin gland, which is significant in size and does not break through on its own after 4-7 days, is removed in two ways: by marsupialization or extirpation.
During the marsupialization procedure, an additional opening is created in the Bartholin gland (a duct is formed) to ensure the outflow of fluid and prevent the formation of a new cyst.
Extirpation of the Bartholin gland involves the removal of the entire gland and is performed in the case of chronic recurrent bartholinitis.
An abscess of the Bartholin gland must be opened and drained to allow the outflow of purulent exudate. The resulting abscess cavity is treated with hydrogen peroxide (3% concentration). An abscess that has opened itself is also subject to antiseptic treatment to prevent conditionally pathogenic microflora from entering the focus . With a chronically occurring abscess, marsupialization or extirpation of the gland is indicated.
Medicinal treatment of bartholinitis is carried out at home or in a hospital. When treating bartholinitis, it is recommended to abstain from sexual intercourse and stay in bed.
