Signs of mesenteric adenitis.
Mesenteric adenitis is an inflammation of the lymph nodes in the mesentery region. From here comes another name for this disease - "mesenteric lymphadenitis" (from the Latin "mesenterium" - mesentery). The mesentery is a type of connective tissue that attaches the abdominal organs to the inner wall of the abdomen. The mesentery is laced with blood and lymph vessels, and nerves pass through it to the internal organs of the abdominal cavity (in particular, to the intestines).
The causes of inflammation of the mesenteric lymph nodes are not fully understood, but there are a number of factors that can provoke such inflammation:
- Specific pathogenic microorganisms - for example, mycobacterium tuberculosis (especially extrapulmonary tuberculosis);
- Nonspecific opportunistic pathogenic microorganisms: bacteria (staphylococci, streptococci, Escherichia coli, Salmonella, Campylobacter, enterococci), viruses (adenoviruses, enteroviruses, cytomegalovirus, Epstein-Barr virus).
For this reason, nonspecific mesenteric adenitis and specific mesenteric adenitis are distinguished, each of which can take an acute or chronic form of the course, and also develop as purulent inflammation.
Infectious agents can enter the mesenteric lymph nodes in several ways: through the bloodstream from the primary foci of inflammation, through the walls of the intestinal tract, through the lymphatic vessels. Since there are several hundred lymph nodes in the abdominal cavity that perform the protective function of the immune system , the defeat of such a large accumulation of lymph nodes is threatening, and therefore it is necessary to recognize the signs of mesenteric infection in time so that the treatment is timely.
Signs of mesenteric adenitis may resemble those of the onset of appendicitis:
- There are paroxysmal abdominal pains in the navel area and below it,
- When turning lying on your back to the left side, the pain point moves from right to left,
- Further, the temperature may rise to subfebrile and higher,
- Signs of nausea and urge to vomit appear,
- Dyspeptic disorders are observed .
Clinical manifestations of mesenteric adenitis can be observed for several hours or days. On examination, the abdominal muscles are not tense, but when palpating, you can feel the density of the lymph nodes. During auscultation (listening), you can hear the rubbing noise of the mesenteric leaves ("rasp symptom"). Diagnosis is confirmed by blood tests with severe leukocytosis, X-ray, identification of sources of infection (purulent foci, viral infections, tuberculosis), biopsy of mesenteric lymph nodes. Mesentery is differentiated from surgical diseases of the abdominal cavity.
Treatment of mesenteric adenitis is complex (drug therapy and surgery).
- Prescribe broad-spectrum antibiotics (cephalosporins and fluoroquinols of the latest generations).
- Desensitizing drugs are used.
- With severe pain symptoms, antispasmodics are prescribed, perirenal blockade is performed.
- They carry out detoxification measures.
- In the case of tuberculosis, as the cause of mesenteric adenitis, treatment is carried out according to the anti-tuberculosis therapy regimen.
- If there is a danger of developing a generalized infection in the case of purulent mesenteric adenitis, an autopsy and drainage of the purulent focus is performed.
In case of suspicion of mesenteric adenitis, without the participation of a doctor, in no case should you use heating procedures (hot bath, heating pad) and take painkillers (they will distort the picture of the development of the disease).
Mesenteric infection can be complicated by abscess, peritonitis, and sepsis. There is a possibility of developing adhesive disease and intestinal obstruction.
